Becoming a Patient
How do I become a patient?
To become a patient you must call our office at (352)-854-5530 and speak to our scheduling department. They will guide you in setting up your New Patient appointment with one of our Providers. You will then be guided to fill out our New Patient Form which is due at our office prior to your appointment. You may drop it off in the office, mail it, or email it. Lastly, you will receive your appointment time and date and are asked to arrive 15 minutes prior to your appointment for any other paperwork needing to be completed. We look forward to having you as a patient at Absolute Health.

What do I need to do before my appointment?
Please arrive 15 minutes before your appointment. We ask that you provide your new patient paperwork before your appointment to verify your information and to provide you with a smooth and fast check-in process. Please bring your photo ID and Insurance Cards and/or form of payment.
Patient Guidelines for Medical Record Submissions
In your New Patient packet, you will find to records form. The Medical Records Release and the Family Medical Release. The Medical Records release will allow you to write down any physician or hospital for which you have seen and sign the release to allow us to gather those records before or after your appointment. The Family Medical Release, allows us to speak to a family member or friend with regards to your care if you choose to provide that. Please note: you can revoke the release at any time you choose; simply speak to one of our staff members for assistance.

We are right off of I-75 exit 350 heading SW on College Road. We are right behind the CVS pharmacy in the Jasmine Plaza. We are also across the street from Walgreens and Zaxbys. If you need assistance with directions please call us at (352)-854-5530. You may also use google maps and enter our address 7350 SW 60th Ave Suite 2 Ocala FL 34476 to find us.
What is the best way to get around Ocala?
You can rent a vehicle from a nearby rental company or you may use Uber or Lyft as some available choices to you. Please note we do not sponsor the use of these services.

Rates and Policies
What are your rates?
We provided a same-day discounted rate for Self-Pay patients who do not have insurance. This must be paid in full same-day to receive the discounted rate. We also provide a good faith estimate.
What is your cancellation/ rescheduling policy?
We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much-needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly “full” appointment schedule.
All appointments must be canceled within 24 hours on a business day during business hours, for same day cancellations, a $30 fee will be due immediately upon cancelation for established patients.
All appointments must be rescheduled within 24 hours on a business day during business hours, for the same day reschedules a $30 fee will be due immediately upon cancelation for established patients.
For all New Patient appointments canceled or rescheduled without 24 hours notice on a business day within business hours. For same-day cancellations/ reschedules a $60 fee will be due immediately upon cancelation.
For Lifestyle appointments, you must provide 48 hours notice during business days within business hours, if so you will be charged a fee of $100 from your deposit.
