- Insurance card(s)
Completed New Patient Packet
If you haven’t had the opportunity to complete the new patient packet beforehand, don’t worry; we can provide you with a copy when you arrive at our office.
What to Expect:
To ensure a smooth experience, we kindly request that you arrive at least 15 minutes before your scheduled appointment time. This extra time allows us to efficiently process your paperwork, update your medical chart, and verify your insurance coverage, if necessary. At the time of your visit, please be prepared to settle any copay, deductible, or coinsurance fees. For self-pay patients, the same-day payment rate will also be due during your visit.
Once all your necessary documents have been processed, our medical assistants will direct you to our exam rooms to gather your medical history and any other pertinent information. Following this, you will have the opportunity to meet with your healthcare provider.
